Pain is designed to protect, not to provide a precise image of tissue. A small injury may hurt greatly in one context, while significant tissue change may produce little pain in another. Imaging findings can also appear in people without symptoms.
As pain persists, sensitivity and threat learning may contribute alongside tissue-related factors. This can make ordinary movement feel dangerous even when the relationship between movement and harm is not straightforward.
The distinction should reduce fear without dismissing experience. “Pain is not always damage” is not the same as “pain never matters.”
For example: a movement can feel more painful when it is unexpected or feared and less painful when it feels controlled, even when the external load is similar. That change does not prove that tissue is irrelevant; it shows that pain has more than one influence.

Evidence
The modern pain definition recognizes pain as a personal sensory and emotional experience influenced by biological, psychological, and social factors. Research on asymptomatic imaging findings shows that structural change and symptoms do not map perfectly, although imaging remains important when used for the right question.
Evidence cannot determine the cause of an individual symptom from a general article. The same sentence may be reassuring in one context and unsafe in another if warning signs are overlooked.
Clinical Perspective
Interpret pain using onset, mechanism, pattern, function, neurological signs, systemic symptoms, and change over time. New trauma, progressive weakness, loss of bowel or bladder control, fever with severe symptoms, or rapid decline requires prompt individual attention.
