Timing and trend are useful but not definitive. A strain, tendon problem, joint injury, or other condition can overlap with ordinary soreness. Pain during exercise can also have multiple meanings.
The safest question is not simply “Does it hurt?” but “What happened, how is it changing, and what can the person do?”
For example: diffuse thigh tenderness that develops the day after an unfamiliar squat session and steadily settles differs from sudden focal pain with a pop, immediate weakness, and rapidly increasing swelling. The pattern informs the response but does not replace diagnosis.
